NHA Driving Equitable Care Toolkit

The Nebraska Hospital Association (NHA) Quality Team created this Toolkit to assist Nebraska hospitals to drive equitable care across the state while also meeting regulatory requirements related to health equity. The content of this toolkit can be applied across states lines and assist Iowa hospitals reach their goals, screen for social drivers of health, and drive quality improvement.
